Use of the South Florida Smile Spa, Nicole M. Berger, DDS text message–based services (the “Messaging Service”) is subject to the following Terms of Service. These terms constitute a legal agreement (the “Agreement”). Please read them carefully.
Program description
South Florida Smile Spa, Nicole M. Berger, DDS uses SMS to communicate with patients about their care and their account with our practice. Messages are sent from our main practice line, (954) 785-1100.
How you opt in
We offer several ways to opt in to the Messaging Service, including entering your mobile number on a form on our website, providing your mobile number on new patient paperwork or a consent form at our office, providing it verbally to a member of our team, or texting our practice line directly. Consent to receive text messages is not a condition of purchase or of receiving any dental service from us.
When you sign up for our Messaging Service, you agree to receive the following types of SMS from us:
- Appointment reminders, confirmations, and scheduling
- Follow-up and post-operative care instructions
- Billing and account updates
- Marketing and promotional messages about our services
Message frequency and cost
Message frequency varies based on your interactions with our practice, including your appointment schedule, treatment plan, and account activity.
Message and data rates may apply for any messages sent to you from us and to us from you. South Florida Smile Spa is not liable for the cost of any such messages. Carriers are not liable for delayed or undelivered messages. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
Opting out
You can cancel the Messaging Service at any time. Just text STOP to unsubscribe. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time or reply START, and we will start sending SMS messages to you again.
Opting out of the Messaging Service will not affect your ability to receive dental care from our practice. We will contact you by telephone, mail, or email as needed regarding your appointments and treatment.
Help
If at any time you forget what keywords are supported, just text HELP. After you send the SMS message “HELP” to us, we will respond with instructions on how to use our service as well as how to unsubscribe. Privacy, health information, and device access
Standard SMS text messaging is not encrypted. You understand that anyone with access to your mobile phone may be able to view the messages you receive when using the Messaging Service, and you agree that South Florida Smile Spa, Nicole M. Berger, DDS will not be liable to you if this occurs. Please notify us promptly if your mobile number changes so that we do not send messages to a number you no longer control.
Do not use the Messaging Service to communicate a dental or medical emergency. If you are experiencing a medical emergency, call 911. For urgent dental concerns, call our office directly at (954) 785-1100.
Authorization
You understand that you are not required to consent to the Messaging Service to receive any other services from South Florida Smile Spa, Nicole M. Berger, DDS.
By agreeing to these Terms of Service and providing us with your mobile phone number when you opt in to the Messaging Service, you authorize South Florida Smile Spa, Nicole M. Berger, DDS to contact you by text message at your mobile phone number using an automatic telephone dialing system or device, or any other computer-assisted technology as applicable.
